Hospice Care Coordination
Holy Care Hospice provides and ensures individualized care that the patient needs. Medical Doctors and a multi disciplinary team will coordinate a Plan of Care that will be discussed with the patient and family to achieve palliation, management of acute medical symptoms that enhance quality of life.
